Per the Affordable Care Act, the Woodstock Foundation must provide the attached notices to all employees regarding 2022 health insurance benefit. The notices provide the eligibility rules for our group health plan, notice of the January 1 open enrollment period, and the contribution the Foundation will make in 2022 toward the health insurance premium.
